My contact forms have been missing for quite some time which is causing me great stress as not only can visitors to my site not contact me I am now also unable to receive any more subscribers? Does anyone else have this problem or what I need to do to re-insert the contact forms.

The theme did not make your contact plugin disappear. Go download a new contact plugin and install it.
